As part of ongoing efforts to improve user experience and enhance IT security, the Identity Management team will be migrating a key system to the cloud from May 20 to 23. There is no impact expected for existing university users, though new student and employee accounts assigned during this time will be created after this maintenance window. ISS and OIT are working with multiple departments and vendors campuswide to implement this migration quickly and effectively. Qualtrics provides a cloud-based survey platform allowing organizations to standardize data and surveys. All faculty, students and staff have access to Qualtrics at no cost.
The university has a Business Associate Agreement (BAA) with Qualtrics and is an approved software provider for forms that collect Protected Health Information. This includes information as part of the Health Insurance Portability and Accountability Act (HIPAA). All new forms that collect Protected Health Information are required to reside in Qualtrics as of January 2021.

Other forms and surveys software to consider are Microsoft Forms, which integrates with Microsoft Teams, and Formstack.
