New Hours for Optional Vaccination Stickers Starting Tuesday, September 7

Stop by the Occupational Health Clinic on the 2nd floor of the Anschutz Health and Wellness Center weekdays to receive a "COVID VAX 2021" sticker for your CU Anschutz badge.
WHEN
- Mondays: 9 a.m. - 11 a.m. and 1 p.m. - 3 p.m.
- Tuesdays: 9 a.m. - 11 a.m. and 1 p.m. - 3 p.m.
- Thursdays: 9 a.m. - 11 a.m. and 1 p.m. - 3 p.m.
WHO
Vaccinated CU Anschutz students, employees and contractors with university badges, who have one of the following forms of vaccination verification:
- COVID-19 Vaccination Record Card
- Photo of COVID-19 Vaccination Record Card
- Medical Immunization Documentation
- Visual Verification of Immunization Record from
- Medical/Health System Portal
WHAT
Optional “COVID VAX 2021” stickers will be applied to your university badge. After presenting vaccination verification, you will simply remove your badge from its holder to have your sticker applied by distribution desk staff, scan your badge, and be on your way.
Please note that there is a vaccination verification process in place for our campus, with a September 1 deadline. Find more information here.
