Insurance ID Cards
Your SHIP insurance ID card provides proof of your health insurance coverage and should be presented whenever you receive medical, dental, vision, behavioral health, or pharmacy services. You can access your digital ID card at any time through your Anthem member account or the Sydney Health mobile app.
How to access your SHIP ID card:
Option 1:
Access your ID card via the Sydney Mobile app
The quickest and most convenient way to access your digital ID card is through the Sydney Health mobile app.
- Download the Sydney Health app from the Apple App Store or Google Play.
- Log in using your Anthem member account.
- Select ID Card to view your digital insurance card.
- Present your digital card at your appointment or pharmacy.

Option 2:
Access your ID card online.
You can also view, print, or download your ID card through your Anthem member portal.
- Visit www.anthem.com
- Log in to your member account.
- Select My ID Card.
- View, print, or download your card.
