Medical & Religious Exemptions to COVID-19 Vaccination Requirement

Students and new employees may request exemptions to the vaccination requirement, per the CU Anschutz COVID-19 Vaccination Policy. Religious exemptions are not available for students. Please note that your exemption request will be reviewed, and you will be contacted by a campus representative to validate that request; until it is validated, you are not cleared to be on campus. 

Unvaccinated individuals with approved exemptions to the campus vaccination requirement are required to wear a mask and observe physical distancing.

Medical Exemptions

Medical exemptions require the signature of a licensed medical provider, so please plan accordingly. Students and employees may download the medical exemption forms using the links below.

Medical exemption from vaccination is only approved for individuals with an absolute contraindication to vaccination per CDC guidance. Currently, the only absolute contraindications to COVID-19 vaccination are i) severe immediate allergic reaction to a previous dose or to a component of the COVID-19 vaccine, ii) immediate reaction of any severity to a previous dose or known (diagnosed) allergy to a component of the COVID-19 vaccine.

Students who are filing an exemption request must submit the exemption form to their school or program, and submit an exemption request using the vaccine verification system. Students can then forward their confirmation email to their school or program.

Employees must submit their provider-signed medical exemption form through the vaccination verification system. Please have your signed form ready to upload in JPG or PDF format as you complete the vaccine verification process.

Religious Exemptions

Current employees may initiate a religious exemption request within the vaccination verification system. There is no form necessary for employee religious exemption requests; you may select this option as you complete the vaccine verification process, and a request for a supporting narrative will be sent to you from the vaccine verification team.

Candidates for employment at the CU Anschutz Medical Campus will be asked by the hiring department’s HR Business Partner to identify requests for religious exemption at the conclusion of the interview process, prior to hiring, so as to ensure the requested exemption can be accommodated. The HR Business Partner will work with the candidate for employment to complete the Candidate Request for Religious Exemption from COVID Vaccination Requirement form.
