Medical & Religious Exemptions to COVID-19 Vaccination Requirement

Students and employees may request medical or religious exemptions to the vaccination requirement, per the CU Anschutz COVID-19 Vaccination Policy.

Medical Exemptions

Medical exemptions require the signature of a medical provider, so please plan accordingly. Students and employees may download the medical exemption forms using the links below.

Students should submit their medical exemption form to their program before beginning the fall 2021 semester.

Employees must submit their provider-signed medical exemption form through the vaccination verification system. Please have your signed form ready to upload in JPG or PDF format as you complete the vaccine verification process.

Religious Exemptions

Religious exemptions require students and employees to attest to their exemption based on religious beliefs.

Students may download the religious exemption form using the link below, and should submit their religious exemption form to their program before beginning the fall 2021 semester.

Employees may claim a religious exemption within the vaccination verification system. There is no form necessary for employee religious exemptions; you may simply select this option as you complete the vaccine verification process.
