NIH R01-NR016209; 7/1/16-4/1/21 ($2,946,919; directs+indirects). The major goal of this study is to determine if an interdisciplinary approach to home health rehabilitation including nutritional supplementation, improves physical function and reduces re-hospitalizations more than usual care. VA Merit Award RR&D I01 RX-001978-01 4/1/16-3/31/20 ($1,000,000). The major goal of this project is to evaluate whether high intensity, front-loading of home health rehabilitation following hospitalization can improve community mobility, quality of life, and reduce caregiver burden in older Veterans.
